School Dental Forms

As a courtesy, school dental forms can be filled out during your child’s scheduled appointment at no additional charge. You are also welcome to email us or call us and request forms to be sent which takes 3-5 days to be completed.

If you require immediate assistance for a school form to be filled out and it is not during your child’s scheduled appointment, there would be a $15 expedited fee while you wait. We cannot guarantee your wait time so please call ahead of time to make sure there is staff available to assist you.
